Though the state of Wisconsin mandated birth registration of its residents in 1852, noncompliance was received until 1878, when the same law was enacted. This time though, people were more compliant up until 1907, when the Wisconsin Bureau of Vital Statistics was established. Hence, if you wish to obtain birth records from the earliest years up to the present, you may contact the Vital Records Services of the Wisconsin State Department of Health. However, these are restricted documents and therefore not considered as public records. Thus, you must present a proof that you are the individual named on the certificate or that you are an immediate family member or a legal representative. Aside from submitting a completed application form, a photocopy of valid government-issued photo ID and the standard fee of $20, you must also enclose a stamped, self-addressed No.10 business size envelope if you prefer to mail your document. Furthermore, you may request for additional copies of birth records requested at the same time by adding another $3-fee for each copy. These payments may also be paid via personal check or money order made payable to the State of Wisconsin Vital Records. Aside from requesting copies of birth records in person, by mail or by fax, you may also opt to process your documents online, wherein major credit cards are accepted.

Remarks: State Office has scattered records earlier than 1857. Records before October 1, 1907, are very incomplete. Additional copies of the same record ordered at the same time are $3.00 each.
