The New York State had started its vital recording of important events, such as births and deaths, ever since the year 1881. If you wish to obtain these significant documents, you may consult the Vital Records Section of the New York State Department of Health. While genealogy research can be performed to uncertified copies of older certificates, the Vital Records Section issues certified copies to qualified applicants. These restricted documents are also indexed in the Local Register of the municipality, where the birth occurred. You may be qualified to obtain such vital documents if your name appears on the record, and if you are an immediate family member (spouse, parents, grandparents, children, grandchildren of legal age) or a legal representative who can present a court order. In addition, you must present valid photo identification, such as a passport, State-issued identification card, Military identification or a driver's license. If you do not possess a photo ID, you may instead present a letter coming from a government agency dated within the last 6 months, like a utility or telephone bill. Once you fail to present any of the mentioned identifications, your request will be disregarded. In addition, you must also prepare a $30-standard fee, which is non-refundable, regardless of the fact that no documents are found upon search.

Remarks: State office has records since 1881. For records before 1914 in Albany, Buffalo, and Yonkers, or before 1880 in any other city, write to Registrar of Vital Statistics in city where event occurred. For the rest of the State, except New York City, write to State office.
