Illinois Birth Records: Discover Your Loved One's True Identity

Birth records are considered part of a state and local government's vital records database, stored for the public to freely examine and use for their own research and information. These records often include copies of official birth certificates, which will provide you with the following information:

  • Date and place of birth
  • Full birth name
  • Full names of parents and their personal identifying information

The state of Illinois started its mandated statewide vital recording in January 1916, though there were earlier records of births that were found. If you wish to obtain copies of birth records prior to January 1916, you may communicate with the clerk of the county where the birth occurred. Documents after January 1916 up to the present can now be found in the Division of Vital Records of the Illinois Department of Public Health. The Vital Records issues birth certificates that are certified copies for $15, while a certification only costs $10. You may also opt to acquire additional copies of birth certificate requested at the same time by adding another $2-fee for each copy. These fees may be paid via money order, certified check or personal check made payable to the Illinois Department of Public Health. For you to apply, you must fill up an application with your signature, together with the standard service fee. You may apply in person, by mail or by phone. Your application must include only a copy of valid photo identification, such as a state identification card, military identification card, passport or driver’s license. You are only legible for a certificate if you are at least 18 years old, your name appears on the record (may you be the registrant or parent), or if you are a legal representative who can present a court order. In addition, you may also process your documents via the Internet and use your credit card as a mode of payment.

Remarks: State office has records since January 1916. For earlier records and for copies of State records since January 1916, write to County Clerk in county where event occurred (county fees vary). The fee for a search of the State files is $10.00. If the record is found, one certification is issued at no additional charge. Additional certifications of the same record ordered at the same time are $2.00 each. The fee for a full certified copy is $15.00. Additional certified copies of the same record ordered at the same time are $2.00 each.
