You may gain access to these documents by communicating with the Bureau of Vital Statistics of the State of Florida Department of Health. This Bureau also issues computerized certificates with an embossed seal on a safety paper. You must first submit a completed application form, a photocopy of govenrnment-nssued photo ID, such as driver's license, passport or State-issued Identification card, and the appropriate fees, which may vary upon your request. If you opt ot obtain a computerized certificate, you must prepare $9, while it will cost you $14 to acquire a photocopied certificate. Moreover, you may request for additional copies of the same document requested at the same time by paying another $4 for each copy. Aside from availing of their walk-in, phone and mailing service (10 days processing), you may also process your documents online, wherein it will only take about 1-2 working days.
